Please remember that several items MUST be on file in our office for a student to practice or tryout.
These include:
- OHSAA Physical Form– including the HIPPA privacy authorization and a risk acknowledgement. Due prior to the first practice. Middle school physicals have been sent to the high schools.
- Emergency Medical Card– Due prior to the first contest.
- Travel Permit– Due prior to the first contest.
- $65 Pay to Play fee (please make checks payable to Hilliard Darby Athletics) – Due prior to the first contest.
